and what is a custom profile infraction?

When i reach 4500 will it change into chatterbox or can i keep the image/gif ?

When i want to change my max sized avatar to something else do i have to pay for it again or do i change it myself in the avatar changing?

Haven't tried since i don't wanna loose le big avatar c:

It can only be changed manually after that. Or if you get banned.

You'll have to pay for each change, yes. A bigger sized image is set using admin tools, the account still has the same restrictions (unlike VIP when the restrictions are changed to 150x150)
