First game that scared me:
DOOM
I was playing late at night with all the lights off.
Some Cacodemon popped out of nowhere and scared the hell out of me.
Also, System Shock freaks me out, but more in a slow mindfuck sort of way.

I tried to play Resident Evil 2 when I was younger, couldn't get halfway through the game without turning the ps off for at least a week. :l
I played through Silent Hill 4: The Room about 4 times, no problem.
Never did finish Siren though. Scary stuff.

Siren is fuckin' scary. I remember playing it one time and I was really focused on the screen then I heard a door screech in my house, I turned around and my brother was there. I shat myself.
