Generally, if the name you wish to change to is already taken by an account, the account must be inactive for at least 3 years.

does this include forum activity? ex. last played 3 years ago, but last forum activity was 1 year ago, but follows all the other namechange rules (qi, posts, etc)

The backend should be checked, I was waiting on a name that was supposed to be up for grabs September of this year but it got snatched up January of this year so I think its just the year date doesn't seem like the month is taken into account
