I'd suggest having a look at other applications to get the general idea of how one is properly done.

As we're a forum-based clan, we don't care for belts of ranks, etc. Just tell us a little bit about yourself (within reason), get to know us in our irc, and like danwebb said: "too much caps"!!! You don't need caps to make your application any more noticeable.

Basically, you need to tell us why we should let you in.
